Eligibility • Patient must have had an adverse event to the vaccine. • CARM (Centre for Adverse Reactions Monitoring) form must be completed. • Observations are required for full assessments. • Full assessments can only be performed by a Nurse Practitioner or GP. If there is any suspicion that the adverse event may have been caused by the COVID-19 vaccination, please report to CARM here.
Key Information • Report any concerning, or serious, event or reaction following immunisation. • Discuss future immunisations and carefully consider any reactions to a previous vaccine dose. If in doubt about safety, seek specialist advice. |

Exclusions • Claiming is limited to one type of claim per person, per day, per practice. • No simultaneous claiming against other funding streams, this includes claims for COVID-19 Community Care Assessments. |

COVID 19 Temporary Medical Exemption

Key Information • Exemptions will be limited to situations where a suitable alternative to the COVID-19 vaccine is not readily available for the individual. • Exemptions will be for a specified time, reflecting, for example, recovery from clinical conditions or the availability of alternate vaccines. • The application may only be on the grounds that the person meets the specified exemption criteria.
Potential examples of COVID-19 vaccine exemption criteria include: • Acute illness—Recent COVID-19 infection, acute moderate to severe illness, or high dose immunosuppressives; or • Significant adverse reaction to previous dose; or; • Pre-existing condition impacting on vaccination. |

Key Information • A primary course, for those over 12 years of age is 2 doses, given at least 3 weeks apart. A prescription and written consent is required. |

Eligibility The eligibility criteria only applies to people who are severely immunocompromised: • Individuals with primary or acquired immunodeficiency states at the time of vaccination; • Individuals on, or recently on immunosuppressive therapy at the time of vaccination; • Individuals with chronic immune-mediated inflammatory disease who were receiving or had received immunosuppressive therapy prior to vaccination; • Individuals receiving long-term haemodialysis or peritoneal dialysis.
Key Information • The third primary dose is available by prescription only and this must be provided by an authorised prescriber who has received informed, preferably written, consent from the patient. • This third primary dose should be given 8 weeks after the second dose, but may be given 4 weeks after, depending on current or planned immunosuppressive therapies. |
